Public School Deferred Compensation Sign-Up

Complete and submit the following registration form to get your reporting unit started in the Deferred Compensation Option through the State of Michigan 401(k) and 457 Plans. For details on the Deferred Compensation Option, go to www.michigan.gov/psru.

By completing the registration form, you are certifying the following:

• You are a school business official, or are authorized by a school business official, of the reporting unit you provide on the registration form; and
• You are signing up to offer the Deferred Compensation Option to eligible employees of the reporting unit listed on the registration form.

Please complete one form for each reporting unit you are signing up.
