Course Fees
Cost of Course includes lunch and the books: Introduction to the FDM and Why Does It Hurt? 
AFDMA Member  - $700
Residents - $450
Active Military - $600
Medical Students (DO/MD) - $250
Nonmembers - $950  (or consider joining AFDMA when you register and save $50 today!) 
Repeat Course Attendee* - AFDMA member - $450
Repeat Course Attendee* - AFDMA non-member - $550

AFDMA is offering a discount for offices with multiple registrants from the same office location. Here is the breakdown:

  • 3 to 4 registrants from same office - $100 off each registration (member or nonmember rate)
  • 5 and over - $150 off each registration (member or nonmember rate)

Discount will be applied after registrations have been verified and depending on the registration type, the appropriate refund will be issued through PayPal back to the registrants from same office location. A waitlist will be available when each category has reached its limit.

Cancellation Policy 
Full Refund on or before 08/26/2016, 50% Refund 08/26/2016 - 09/09/2016.  No refund after 09/09/2016. However, course fee can be applied to future course. If course is cancelled by AFDMA at any point, a full refund of course registration will be offered.

Hotel Reservations
AFDMA has not established a room block at this hotel or any hotel in Detroit allowing registrants the opportunity to stay anywhere. It is highly recommended that a hotel reservation be made as soon as you register and hotel rooms are sometimes very hard to come by in Detroit and surrounding area. 

CME Hours
The AFDMA has requested that the AOA Council on Continuing Medical Education approve this program for 20 credits of AOA Category 1A CME credits.  Approval is currently pending.

The AFDMA has requested that the American Academy of Family Physicians approve this program for 19.00 Prescribed credit(s).  Approval is currently pending. 

This activity has been approved by the Texas Board of Physical Therapy Examiners for 18.75 CCUs for PTs and PTAs.

All other healthcare professionals must submit their seminar certificate to their licensing organization to receive CEUs.

Physicians should claim only the credit commensurate with the extent of their participation in the activity.
