Cancellation Policy
Full Refund on or before 05/13/2016, 50% Refund 05/14/2016 - 05/27/2016.
No refund after 05/27/2015. However, course fee can be applied to future course.
Hotel Reservations
AFDMA has not established a room block in East Lancing allowing registrants the opportunity to stay anywhere.
CME Hours
The AFDMA has requested that the AOA Council on Continuing Medical Education approve this program for 20 credits of AOA Category 1A CME credits. Approval is currently pending.
This AFDMA program has been approved by the American Academy of Family Physicians for 19.00 Prescribed credit(s).
All other healthcare professionals must submit their seminar certificate to their licensing organization to receive CEUs.
Physicians should claim only the credit commensurate with the extent of their participation in the activity.
