Nonmembers - $950 (or consider joining AFDMA when you register and save $50 today!)

A waitlist will be available when each category has reached its limit.
Cancellation Policy
Full Refund on or before 6/25/2015, 50% Refund 6/26/2015 - 7/10/2015.
No refund after 7/10/2015. However, course fee can be applied to future course.
CME Hours
The AFDMA has requested that the AOA Council on Continuing Medical Education approve this program for 20 credits of AOA Category 1A CME credits. Approval is currently pending.
The AFDMA has requested that tthe American Academy of Family Physicians approve this program for 19.00 Prescribed credit(s). Approval is currently pending.
Physicians should claim only the credit commensurate with the extent of their participation in the activity.
