Course Description

Partnership is the foundation of Patient & Family Centered Care (PFCC).  Relying on the core concepts of PFCC to establish partnerships across healthcare settings provides an opportunity for patients and families to impact processes from the bedside to the boardroom.  The 5th Annual PFCC Conference will showcase diverse examples of  powerful outcomes achieved through fostering partnerships with patients and families  to improve the quality, safety and experience of healthcare. The goal of this year's conference is to provide attendees with resources, tools and practical strategies for engaging patients and family members across all aspects of healthcare delivery.  Attendees will leave the conference inspired, encouraged and equipped to advance PFCC in their own organizations.

Target Audience

An interdisciplinary approach is a central component of Patient & Family Centered Care. Involvement of all members of the care team is imperative: patients, family members, parents, allied health care professionals, physicians, nurses, therapists, social workers, spiritual service, palliative care representatives and hospital administrators. Interdisciplinary teams from organizations are encouraged to attend.

Learning Objectives
At the conclusion of this activity the participant should be able to:

  • Identify best practices, resources and models for engaging patients and families in their own care and for building collaborative partnerships to improve the quality, safety and experience of care.
  • Discuss ways to maximize the impact of patient and family advisory councils.
  • Describe strategies for integrating PFCC practices into organizational systems and culture.
  • Recognize the competencies, organizational resources and support needed for successful engagement of patients and families.

ACHE Credit: This program has been approved for 6 hours of pre-approved ACHE Qualified Education credit (non-ACHE) for this program toward advancement, or recertification in the American College of Healthcare Executives.  Participants in this program wishing to have the continuing education hours applied toward ACHE Qualified Education credit should indicate their attendance when submitting application to the American College of Healthcare Executives for advancement or recertification.

BRN Credit: Provider approved by the California Board of Registered Nursing.  CEP #970 for 6 contact hours.
