Qualifying premium applies to policies written and placed between 9/1/2014-3/31/2015. Annuity premium & Single Premium products are weighted at 5%, excluding rop products. Invitations will be extended at the end of the contest period at the sole discretion of Levinson & Associates. Policies must be in good standing. Incentives apply only to licensed, writing agents appointed directly with Levinson & Associates. MGA & GA' qualifications may vary Attendees will receive a Form 1099 for the value of the trip.
