Central Indiana Case Management Society of America (CICMSA) invites you to participate in our 21st Annual Fall Conference.
 We encourage you to join this energetic group of professional Case Managers.
CICMSA is the foremost association of professionals dedicated to promote the growth and value of case management and to support the evolving needs of the case manager. This conference provides an opportunity to network with other professionals and reach case managers from acute care, long term care, workers’ compensation, insurance and individual companies.
Silver Sponsorships which include a table in our exhibition area are still available for the conference. All other levels of sponsorship are sold out. Please select the number of sponsorship tables you wish to obtain below. You may have more than one table as a sponsor which you may designate for different programs or facilities within your parent company. Each table will cost $450 as a Silver Sponsorship table.
Space is Limited:
- To be listed in the Printed Mailing Conference Brochure registration forms and payment must be received by July 31, 2015.
- To be listed in the Conference Booklet, registration forms and payment must be received by September 25, 2015.

Sponsor the lunch and have your name prominently displayed on each luncheon table and one piece of your company’s promotional material at each attendees’ plate, your logo and link on CICMSA home page of the website for one (1) year, and recognition in conference publications. Diamond Sponsorship ~ $1,000
As the Diamond Sponsor your logo will be prominently displayed on the lanyards provided to every attendee, prime location of your 8 ft display in Exhibitor Area, your logo and link and on CICMSA home page of the website for one (1) year, and recognition in conference publications. Title Sponsorship ~ $2,000
