CHMG is offering flu (primary care) & COVID-19 vaccine (all CHMG patients) appointments to existing patients seen within the past three years. Once you select a date and time, a member of our team will provide confirmation of the appointment by phone or email. Please note you ARE permitted to get the flu and COVID vaccine at the same appointment.
