Tournament Registration

Date: May 23, 2016
Time: 11:30 a.m. Lunch, Driving Range opens 
1:00 p.m. Shotgun Start
Format: 4 person Scramble
Registration Fees: $150 per player, $600 per foursome (inc lunch, bev, dinner), $50 dinner only
Awards: Top 3 Teams,  Closest-to-Pin, Longest Drive, Straightest Drive
Cocktails: 5:30 pm (cash bar)
Awards Dinner: 6:00 p.m., prizes awarded.

Entries must be received by May 12, 2016

Intern Scholarship Application

Sponsorship Opportunities: 

Gold Sponsor - $2,500 - two foursomes, hole sponsor, event signage, contribution to scholarship fund

Silver Sponsor - $1,000 - one foursome, hole sponsor, event signage, contribution to scholarship fund

Lunch Sponsor - $750 -Company representative greets all players & hands out sponsor provided goodies at carts, banquet room sign & event signage

Bronze Sponsor - $500 - one player, hole sponsor, event signage and contribution to scholarship fund

Photo Sponsor  - $500 -company representative greets all players while taking photos; event signage,opportunity to provide frames for photos.

Hole Sponsor - $150

Door Prizes - please provide details to SFMA

Goodie Bag Items - Sport towels, hats, balls, tees, etc.

Contributions to the SFMA Golf Tournament are not deductible as a charitable contribution for Federal Income Tax purposes, but may be deducted as an ordinary business expense.

Foursomes or Sponsorships?  Please add each golfers name as "guests" and click on the blue "add another" button to add all golfers.  If you don't know golfer's name yet, just enter golfer 1, 2, etc.
