This completed form along with payment must be received at the CCAH office no later than October 2nd, 2023. If you would like to pay via credit card, the additional costs have been added to the online ticket price.

If you are paying via check the cost of the tickets should be made out to the prices listed below. Any registrations after October 2nd will be subject to Late Registration pricing and there is no gurarantee your preferred meal option will be available.

Once this completed registration form and payment have been received at the CCAH office a confirmation email will be sent to the above email address. Please double check your email before sending information, otherwise we are unable to contact you. If paying by check instead of online payment, you Must send in a physical check with a completed form to hold your seat no later than October 2nd, 2023. Our registration forms for checks are on our website. Payments via credit card are subject to transaction fees. Checks  should be made out to the prices below.

NO REFUNDS will be made after October 16^th. 

Cancellations made after a registration form has been sent but before payment has arrived after the stated deadline will still be subject to being charged.

If you are not sure if you have a membership confirm your 2023 company membership by calling or emailing the CCAH office PRIOR to submitting payment. All registration cancellations must be received by email to ccah@californiacouncil.com. There will be NO refunds made after October 16^th, 2023, however you may send a substitute from the same company and must notify CCAH by email.
