This class will go over the Home Health Regulations for a Medicare Agency. It will talk about skilled need, homebound, face to face - the essential foundations of home healt. Will introduce participanta to the regulatory bodies - CMS /Medicare, States, Accreditation Organizations, MAC's. Also, clinical assessment and documentation requirements and tips will be give. This is an informative webinar for ny clinician new or needing a refresher to the essential foundations of home health.

The purpose of this activity is to: Assis the clinician in understanding the foundations needed in a Medicare certified Home Health agency, including Medicare qualifiers for home health, skilled need, clinical documentation and more.

OASIS C-2: Ensuring Accuracy and Consistency

The outcome and Assessment Information Set (OASIS) assessment is constantly evolving to meet the latest regualtions from Medicare. The assessment is intended to assess patient's health status, needs and aide in the development of an individualized plan of care. OASIS accuracy and consistency within a home care agency are critical to ensure quality patient care and imporved patient outcomes. This session will assist clinicians in accurately completing the OASIS and provide item-specific guidance such as item intent and response instructions.

The New Home Health Conditions of Participation

The Home Health Conditions fo Participation (CoPs) are the RULE book that has to be followed in order for home health agencies to operate. They had not been updated in 3 decades until January 2018! The changes to the CoPs are significant! Surveyors ensure that an agency is compliant to all the Conditions and Standards in the new CoPs. So as a new clinician to home health or as someone who needs a refresher to these new regulations, this is a perfect time to get training - you can't follow the Rules if you don't know them!

Go over all of the 16 Conditions with focus on the clinical ones- Comprhensive Assessment, Quality of Care / Care Planning/ Coordination of Care, Patient Rights, Infection Control and QAPI. We will discuss the New CoP Philosophy- Interdisciplinary team management, Patient Centered, Data Driven- we will help you to ensure you understand what your role is for each of these critical conditions.

The purpose of this activity is to: Teach the participants the new home health conditions fo prticipation that were implemented in January 2018

Caring for the Patient Using an Interdisciplinary Approach

Effective case management is dependent upon the interdisciplinary team working together towards collaborative goals and coordinating the patient care in a proactive manner. Effective case management can enhance patient outcomes by planning a course of interventions and developing a plan to achieve the goal. This session will also show how to complete an accurate comprehensive patient assessment in a standardized manner throughout the entire interdisciplinary team.

The purpose of this activity is to: Understand the Coordination of Care CoP and understand goal driven care and effective case management.

Melissa Abbott RN, MSN, MHA, COS-C

 

Melissa is a registered nurse with experience in a variety of health care settings with the majority of her nursing experience in home health care and hospice where she was a clinician working in the field and then later became an administrator of a hospital-based home health and hospice agency. In 2017, she joined 5 Star Consultants as a home care and lead hospice consultant.
Melissa has an expertise in identifying issues, solving problems and implementing action plans that are successful. She also has a passion for individual coaching, team building and leadership development.
Abbott has served on the Iowa Alliance in Home Care Board of Directors and the Hospice and Palliative Care Association of Iowa Board of Directors since 2014 and has been a part of advocacy work for home health and hospice within her home state of Iowa as well as nationally. 
Melissa was one of the members chosen to sit on the strategic planning committee for the National Association for Home Care & Hospice in early 2018 and is a member of NAHC’s Regulatory Affairs Committee. Melissa also has presented for many state associations at annual conferences along with webinars on a variety of regulatory and clinical topics that pertain to home care and hospice.

Vicky has diversified knowledge and experience expanding over 25 years with national leaders in home health care.

 Vicky’s successes started as a clinical field nurse and expanded to Corporate Consultant. She is a former ACHC Surveyor.  She consults home care agencies on QAPI, regulatory compliance, and survey preparedness.

 Vicky is an excellent educator and is able to teach your staff with easy to learn methods.  She resides in Florida.
