WEBINAR: Outcome Enhancements of Your Home Health Agency in 2018

Improving patient outcomes is a focus area for many agencies but how does one do that? Understanding reports, such as CASPER and Star Ratings, are key to improving patient outcome measures along with OASIS accuracy and consistency among clinicians. This session will assist in analyzing and interpreting your agency reports and provide clinicians OASIS item-specific guidance such as item intent and response instructions.

The purpose of this activity is to:

  • Learn how to look at your CASPER reports and be able to understand the outcomes being measured and how to analyze those outcomes so that you know what areas are a top priority to focus on within your agency.
  • Be able to better assess patients and understand how to appropriately answer the OASIS questions so that it best reflects the status on admit / discharge and see the importance of these items in improving patient outcome measures.

Target Audience: Executives, Management, Clinical Staff

Objectives:

 1.    Increase understanding of CASPER reports

  • Discuss where results come from
  • Discuss time frame that data is collected
  • Discuss the February 2018 changes to the reports
  • Discuss the outcome reports:
  • Agency Patient Related Characteristics
  • Risk Adjusted Outcome Report
  • Potentially Avoidable Events (PAE)
  • Process Measures Rep

2.    Understand the 5 Star Ratings and Home Health Compare

  • Review what measures are reported on HHC and which are used in Star Ratings
  • Review the importance of improving outcomes in relation to star ratings and HHC

3.    Increase knowledge of how to appropriately assess patients and answer OASIS items that will improve your outcomes

  • Review key M items in the following categories along with their intent:
  • ‘Managing Daily Activities’
  • ‘Managing Pain and Treating Symptoms’
  • Preventing Harm’
  • Preventing Unplanned Hospital Care’
4.    Enhance understanding of how to select the outcome measures to focus on
  • Review the best outcomes to focus on within your agency
  • Provide an example on a QAPI indicator for reducing emergent care

Melissa Abbott RN, MSN, MHA, COS-C

Clinical Home Health and Lead Hospice Consultant at 5 Star Consultants

Melissa is a registered nurse with experience in a variety of health care settings with the majority of her nursing experience in home health care and hospice where she was a clinician working in the field and then later became an administrator of a hospital-based home health and hospice agency. In 2017, she joined 5 Star Consultants as a home care and lead hospice consultant.
Melissa has an expertise in identifying issues, solving problems and implementing action plans that are successful. She also has a passion for individual coaching, team building and leadership development.
Abbott has served on the Iowa Alliance in Home Care Board of Directors and the Hospice and Palliative Care Association of Iowa Board of Directors since 2014 and has been a part of advocacy work for home health and hospice within her home state of Iowa as well as nationally. 
Melissa was one of the members chosen to sit on the strategic planning committee for the National Association for Home Care & Hospice in early 2018 and is a member of NAHC’s Regulatory Affairs Committee. Melissa also has presented for many state associations at annual conferences along with webinars on a variety of regulatory and clinical topics that pertain to home care and hospice.
