Team Up, Take Action: A Conference on Partnering to Improve Community Health

Now in its 4th year, the Team Up, Take Action conference convenes public health and health care professionals, community activists, and health advocates to advance best practices and to build collective capacity to improve health and health equity through partnership. The 2019 conference theme is Building Bridges between Communities and Healthcare.
