763. The Health Home: Linking Services for Medicaid-Eligible Users of Medical & Community Based Svcs

New York's Health Home, part of the Medicaid Redesign Team's (MRT) multi-year action plan, is a care management model that serves high cost Medicaid members by linking the member to medical and behavioral health services, housing and community and social supports. This program will highlight two Health Homes operating in the Capital Region.
