"Communication, Case Management & Care Coordination for the COPD Patient " 2.0 CE

PROGRAM DESCRIPTION:

According to Centers for Disease Control and Prevention (CDC) data, approximately 6.4% (around 15 million) of U.S. adults have been told by a health care provider, that they have COPD.  The prevalence and burden of COPD continues to increase due to continued exposure to COPD risk factors and the changing age structure of the world’s population, with more people living longer and therefore expressing the long-term effects of exposure to COPD risk factors.

Case Managers are key to assisting COPD patients and their family caregivers in understanding the required treatment options, risk factors, medication management and development of self-management skills that help to reduce avoidable hospital readmissions.  This session will highlight not only important case management interventions but the key strategies for improving transition planning and care coordination. 

Objectives:

1)      Define the disease burden and at-risk factors of the COPD population

2)      Review Treatment & Case Management Strategies

3)      Discuss adherence challenges and tools for COPD

4)      Evaluate transitions of care planning issues and coordination 


ABOUT OUR SPEAKER:

 Kathleen Fraser, MSN, MHA, RN-BC, CCM, CRRN, the Executive Director of CMSA and also a Past President of CMSA National, and of the Houston/Gulf Coast CMSA Chapter. She has 36 years' nursing experience, 24 years of case management experience and was named the 2016 CMSA National Case Manager of the Year.  Kathy is the author of the CMSA Case Management Medication Adherence Guideline for COPD.
