2016 Fundraiser Request Form 

This form must be completed and submitted electronically to the Muslim American Society of Minnesota. Permission for collection will only be given after all completed documents are presented and full enquires are successfully approved. The Muslim American Society of Minnesota reserves the right to reject an application in the best interest of the Society. The Muslim American Society of Minnesota requires a minimum of two (2) weeks confirming the booking.

NOTICE:

• The Muslim American Society of Minnesota does NOT allow ANY funds to be collected at our facilities or events by organizations unless they are registered with the IRS.
• The Muslim American Society of Minnesota does NOT allow ANY funds to be collected at our facilities or events by organizations unless they are incorporated in the United States.
• The Muslim American Society of Minnesota does NOT send any money outside the United States.
• The only way to collect money in our facilities for foreign relief activities is through a relief rganization incorporated in the United States and approved by the IRS as a Tax Exempt organization.

PLEASE NOTE: We receive a lot of requests. We only approve about 20% of the requests we receive. Submitting this form does not mean that you will be automatically permitted to raise funds. Please DO NOT make plans for the fundraiser until you receive explicit approval of your request.
