Before you begin registration, please read the following instructions carefully.

1. This form is only your Certificate of Election for Lay Deputies. Attendees still need to register apart from this form.

2. Each person must have their own unique email address. Clergy/deputies may not share email addresses. If you need assistance in setting up an email account, please contact the Diocesan Office.
