Do you want to live in your current home as you age? As life changes around you, the barriers to continuing to live in your home may seem insurmountable.

This forum will help older adults, family members, and caregivers learn about the services available to make it possible for an older person to return home after a hospitalization and stay in his or her home longer.

Community agencies will present information and answer questions about their services. Representatives from the Senior LInkage Line will provide information about how people can locate these services in their own communities.

Registration is appreciated but not required.
