Please complete this online form to begin the application process for the April 30, 2020 DACBSP® practical exam and / or the May 1, 2020 CCSP® and / or  DACBSP® written exams. Please know that your application package will NOT be complete UNTIL your transcript, healthcare provider level CPR card, paid registration and DC license are also received by our office.  Your state license and CPR card must be submitted prior to the application deadline AND these documents must still be valid on the date of the exam.

If you are paying via check, the check must be received by the national office prior to the application deadline date. The application deadline is Tuesday, March 17, 2020.

Those using a MS degree as eligibility to sit for an exam, please know that your transcripts will be reviewed on an individual basis for approval and it is possible that a hands-on Emergency Procedures course may be required if a prior one is deemed insufficient.

***A Principles and Practices Workshop is held Thursday, April 30th from 8:00 am - 6:30 pm - see course description below.  This course requires a seperate registraton form.  Click HERE to register! ***

The Principles and Practices Workshop is a fantastic opportunity to learn clinical pearls from some of the leaders in the ACBSP™.  The topics covered throughout the day include management of spinal conditions, concussion evaluation and management, emergency procedures, exercise physiology, and evaluation and management of upper and lower extremity conditions.  The knowledge gained during this workshop will not only assist in post-graduate work but can also be applied in-office and on-field.  This workshop is a great way to take your skills to the next level.
