2024-2025 Care Coordination Certificate Course

This course is designed to provide health professionals, such as Nurses, primary care clinical staff, and alike, to improve care coordination for patients with chronic conditions, and behavioral health issues, as well as incorporate health promotion into their practice. Cost is for ICAHN members is $500 and for non-members is $550.
