Beach Bash 2016

The Glencoe Junior High Project and Glencoe Park District are proud to present the Third Annual Beach Bash! All Central School students are invited to the Glencoe Beach to celebrate the end of the school year. Music will be provided by FAMOS! DJ Entertainment. Students will not be allowed to participate without a signed waiver by their parent/guardian. By pre-registering for the event, you are agreeing to these terms and a completed registration will count as a signed waiver. Those who do not pre-register will be required to have a parent come sign a waiver in person on the day of the event and pay $20 for admission.

Please read this form and be aware that in signing up for and participating in this Beach Party activity, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which your minor child(ren) or you might sustain as a result of participating in any and all activities connected with and associated with said program/activity. Depending upon the specific activity, certain risks, dangers and injuries due to acts of God, inclement weather, slips and falls, inadequate or defective equipment, failure in supervision or instruction, premises defects, horseplay, carelessness, lack of skill or technique, and all other circumstances inherent to the particular activity exist. In this regard, it must be recognized that it is impossible for the Glencoe Park District and GJHP to guarantee absolute safety.

I recognize and acknowledge that there are certain risks of physical activity to participants in this program/activity, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child(ren)/ward(s) or I may sustain as a result of said participation. I further agree to waive and relinquish all claims I, or my minor child(ren)/ward may have (or accrue to me or my child(ren)/wards(s) as a result of participating in this program/activity against the Glencoe Park District and GJHP including its officials, agents, volunteers and employees.

I have read and fully understand the above important information, warning of risk, assumption of risk and waiver and release of all claims. If registering on-line or via fax, my on-line or facsimile signature shall substitute for and have the same legal effect as an original form signature. Photos are periodically taken of participants in a class, during a special event or at the District’s parks and facilities. Please be aware that these photos are for Park District or GJHP use only and may be used in either of their publications.

In the event of an emergency, I understand that patrons may be notified by cell and/or home phone.

Neither the Park District staff nor the Glencoe Junior High Project staff or volunteers will be monitoring students’ departures. This event runs from 2:30pm – 5:30pm. After 5:30pm, there will be no adult supervision.
