Waiver and Indemnity Release: In recognition of the risk of injury while participating in STIGMA*STRIDE, and as consideration for the right to participate in the Event, I knowingly and voluntarily enter into this Waiver and Release From Liability Agreement and hereby waive any and all rights, claims, or causes of action of any kind whatsoever that arise out of my participation in the Event. This is binding on all heirs, executors, administrators, assigns, volunteers, or personal representatives. I hereby release and forever discharge Association for Pelvic Organ Prolapse Support (APOPS) and STIGMA*STRIDE officers, directors, employees, agents, representatives, successors, volunteers, affiliates, sponsors, and assigns from any and all liability, claims, demands, damages, actions, or causes of action now existing or which hereinafter may arise as a result of my participation in the Event, whether any injury was caused by negligence of hosts, myself, or third parties, the conditions of the course, or any other cause. I agree to indemnify and hold harmless Association for Pelvic Organ Prolapse Support (APOPS) and STIGMA*STRIDE against any and all claims, suits, or actions of any kind for liability, damages, compensation, or otherwise brought by me or on my behalf including attorney fees, if litigation arises related to claims made by me or anyone on my behalf. 2.Physical Fitness: I attest that I am physically fit and have trained sufficiently for the Event, and am medically and physically in good health. I also assume all risks associated with participating in this Event in including but not limited to falls, contact with other participants, the effects of the weather including high heat and/or humidity, traffic and other conditions of the road and/or walk course, all such risks being known and acknowledged by me. 3. Media release: I recognize that various photographs, video recordings, and other media will be captured during the Event by Association for Pelvic Organ Prolapse Support (APOPS), STIGMA*STRIDE, and/or local television or radio media agents. I agree to give permission to use any form of media including photographs, motion pictures, recordings, quotations, or any other record of this Event for any legitimate purpose including advertising or promoting worldwide perpetuity in all forms of publications. I, the walker in STIGMA*STRIDE, hereby declare that I have read, fully understand, and acknowledge that by printing my name below, I agree to all aforementioned terms and conditions contained in this authorization to participate in STIGMA*STRIDE.
