Von Miller's Vision Day Minnesota in partnership with The Sanneh Foundation 

Von's Vision provides city-wide vision day for middle- to low-income children. Each child receives a free eye screening and if needed, a free eye-exam and corrective eye wear. The eye screening and exam will occur on the day and time listed.

Before registering on behalf of a student, please print off these forms and fill them out. Each student will need to have the Patient History Form and the Parental Consent Form completed before they can participate in Von's Vision Day. If you are not bringing your child to the program, please ensure they have a printed and signed copy of the attached forms before they leave for the program. In the instance that a student does not have a parental signature on the form, a supervisory adult from the student's organization can serve as signatory on the forms.
