Leadership Optometry - Class IV 

APPLICATION FOR CLASS OF 2016 

Leadership Optometry is a statewide program to select, challenge, develop and educate those involved in the optometric profession, who have demonstrated an interest in local or statewide issues.  The program is designed to be a series of issue-oriented experiences, which are based on the belief that knowledge is a key element and prime motivator of leadership.

 Leadership Optometry is open to all OAOP members.  Participants are selected on ability, interest in their community or profession and insight for effective leadership. 

COMMITMENT 

The course consists of five (5) separate day sessions.  Classes are held in a location that best relates to the topic studied.  As with all worthwhile endeavors, this program requires a definite commitment of time on the part of each participant.  (Spouses and children are not included in meetings, with the exception of graduation.)

Attendance is mandatory for successful completion of the program.  Absence from more than one full session or accumulation of partial sessions will make the participant ineligible to graduate.

Tuition for the Leadership Optometry program is $100.00 which covers a portion of the program costs, meals and site transportation if needed.  Participants must pay their own transportation and lodging expenses.

2016 DATES 

Beginning April in conjunction with the OAOP Vision Summit with day sessions in May, June/July, August/September, and final class session, project presentations, and graduation will be at the OAOP’s Fall Conference.

April 28

May 10

June 18

August 12-13

September 29-30 (30th is graduation)


Completed applications must be submitted by January 15, 2016
