Transition Assessments: The What, How, and Why

Assessment data serve as the common thread in the transition process and form the basis for defining goals and services to in included in students’ Individualized Education Plans (IEPs). This event will review the What, How, and Why of the assessment process, provide guidance in the selection of appropriate instruments and tools, and cover how to conduct an age appropriate transition assessment.

By the end of the session participants will be able to answer the following questions: 

• What is transition assessment and its purpose?
• Why conduct transition assessments?
• How to select and conduct instruments and methods
• What to do with assessment results
