Eligibility Requirements

 Must be a member of Hemophilia of Iowa, Inc. (HOI) through December 31, 2019. Memberships are on a calendar year schedule. Your membership must be submitted on or before March 15th, 2019 when scholarship applications are due. If membership is not submitted on or before March 15th, 2019 your scholarship will not be considered. Do not send your HOI membership application to the scholarship committee, please go online (www.hemophiliaofiowa.org) to access the application.  NO EXCEPTIONS.

  • Member must reside in the HOI service area (State of Iowa or if outside the State of Iowa, must seek care at the UIHC Treatment Center) unless prior arrangements are made.    
  • Must have a diagnosed bleeding disorder or be an immediate family member (sibling, child, or caregiver) of someone that has a diagnosed bleeding disorder (diagnosed bleeding disorder is defined as a person who is being followed with a treatment plan).
  • Verification of diagnosis may be required from your physician upon request (you will be notified if this is required).
  • High school graduate before or in 2019.
  • Be enrolled in a degree seeking program at an accredited college, university, or trade school in the United States during the 2019-2020 academic year for a minimum of 12 credit hours or considered full time.
  • Either you or a representative of your immediate family must be present at the Scholarship Award Ceremony to be held during the Annual Education Weekend (unless agreed upon emergency).

New Requirement

·Must attend one (1) Hemophilia of Iowa event that is NOT THE ANNUAL MEETING during the 2019 year
  • EXCEPTION: Incoming freshmen are not required to attend an additional event

  • The David B. Brustkern Scholarships are awarded to persons that have supported the mission of Hemophilia of Iowa, Inc.

 Incentive Program

Add extra funding to your personal scholarship provided by Hemophilia of Iowa!

We invite all scholarship recipients to participate in selling the Scholarship Fundraiser tickets at face value. The scholarship recipient will receive half of the total tickets sold towards their scholarship.

 The Scholarship Fundraiser is a family fun fundraiser event where all funds go strictly to HOI’s Scholarship Program. Fun activities, great food and entertainment is in store for you! 

 HOI hopes is excited for the involvement of scholarship recipients for the  Scholarship Fundraiser. Our hopes are to continue to strengthen the relationships within the bleeding disorder community.

Application Preparation

You must complete the online scholarship application.

Please submit a minimum of ONE of the following documents:

  • A typed summary of not less than one paragraph and no more than one page of your short range and long range career plans.
  • A typed summary of not less than one paragraph and no more than one page of your personal background related to the bleeding disorder community. 
  • A typed letter of recommendation from a HOI board member, member of Executive Committee, a Committee Chairperson, or someone from the HTC verifying contributions made to HOI and/or support of our mission.  This is optional and could possibly qualify you to be considered for a named scholarship, which is awarded in addition to the scholarship award.     

David B. Brustkern Scholarship

 Application Requirements

 You only have to submit the online scholarship application once (please do not resubmit). Choose either the Dude Cremer or John Heisner Scholarship to apply for. Title Essay with chosen scholarship. Submit the following:

  • A typed essay (one page) on how you support HOI’s mission OR how HOI has impacted your life
  • A typed letter of recommendation from a HOI board member, member of Executive Committee, a Chairperson, or someone from the HTC verifying contributions made to HOI and/or support

Complete online application found on the HOI website before or on March 15th, 2019 to be considered

**if applying for the David B. Brustkern Scholarship supporting documents must be turned in by April 1st, 2019**

All selection decisions are final and are not subject to appeal.  Applications and supporting documents become the sole property of Hemophilia of Iowa, Inc. and cannot be returned.  It is your responsibility to inform the Committee Chair Shane Kelley of any changes to your application.  This includes, but not limited to, change of school and full- or part-time status.
