Year Long registration for participation on QC Am Tour.

PLAYER DISCLAIMER:

For and in consideration of my participation in the Quad City Amateur Tour (hereinafter referred to as “QCAT”) I, _________________________________________ (PRINT full name) hereby, for myself, executors, insurers, successors, heirs administrators, assume any and all risks associated with my voluntary participation in QCAT which includes any and all activities staged by QCAT and unconditionally and voluntarily release and waive any and all rights and claims that I may now or at any time in the future, have against QCAT and their respective subsidiaries, agents, owners, employees, directors, officers, representatives, contractors, sponsors, affiliates, without limitation including all affiliates, successors and assigns (collectively, the “Released Parties”), arising out of my participation in QCAT and/or any related activities, such as activities staged or sanctioned by QCAT (collectively, any and all events). I hereby fully release and discharge the Release Parties named above from any and all claims resulting in injuries, death, damage or any form of loss which I may have or which may accrue to me from my participation in QCAT and/or associated events, even though that liability may arise out of my own negligence, carelessness or other inappropriate or negligent conduct on the part of the above named Released Parties. I further agree to indemnify and hold harmless and defend the above named Released Parties from any and all claims, damage, liabilities, injuries, medical costs, attorney’s fees arising out of, connected with or in any way associated with my participation in any event associated with QCAT, I acknowledge that the Released Parties are not responsible for any inclement weather which may affect delay, postponement or cancellation of any QCAT scheduled event and I hereby waive and release all claims against QCAT and all of the above named Released Parties from any damage, liabilities, loss or injury that may arise therefrom.

This document is intended to be as broad and inclusive as permitted by law, and if any portion thereof is held invalid, it is agreed that the balance shall notwithstanding, continue in full legal force and effect. This document embodies the entire agreement with respect to these matters and supercedes any previous or contemporaneous negotiations or agreements. Each of the readers agrees that no promise or inducement has been offered except as herein set forth under all voluntary and willing agreements and that this document has not been executed based upon statements or representation by any person or entity other than those set forth therein. I have been advised to contact an attorney prior to execution hereof. This document shall be governed by and construed under the laws of the States of Iowa without regard to conflicts of law principles therein.

I further grant permission to QCAT to use a photographic image of myself, without remuneration to me, as I participate in sporting an other events in association with QCAT. My photographic image is for the exclusive usage of QCAT web site, brochures and other promotional materials and may not be sold, used for any other purpose or be distributed in any other venue except as stated above.

I HAVE READ, FULLY UNDERSTOOD AND VOLUNTARILY EXECUTE THE ABOVE WAIVERS, RELEASES, ALL CLAIMS AND UNCONDITIONAL INDEMNIFICATIONS OF THIS DOCUMENT. I ALSO AGREE THAT I WILL ABIDE BY ANY AND ALL EVENT RULES AS STATED ON THE QCAT WEBSITE.
