Curriculum 2: Applied Care Coordination and Systems Navigation

Homeless Health Care Los Angeles is excited to announce no cost, regional training opportunities in collaboration with LAHSA that are available throughout Los Angeles County.

Curriculum 2: Applied Care Coordination and Systems Navigation - Cohort D

Course Description: 

Designed for direct service staff with a minimum of 6 months experience in homeless services, this course offers a more applied approach to engaging the core tenets and practices outlined in the Care Coordination and Systems Navigation curriculum. Through lecture and interactive exercises, this course draws upon trainees’ experience in the field to explicate training topics and apply them to actual scenarios that direct service staff have encountered, or may encounter, as they care for members of the homeless community. This course consists of 5 full-day trainings, which are 7 hours each. Participants will receive an Applied Care Coordination and Systems Navigation Certificate upon completion of the 5-day course. 35-hour CEUs are available for this course from BRN, CAADE, CAMFT, CCAPP.


This five day, 35-hour course will instruct the following topics:

Day I – The Homeless Services Delivery System, Core Functions of Case Management, and Crisis Management 

Day II –Boundaries & Ethics, HIPAA, and Culture 

Day III – Physical Health and Mental Health 

Day IV – Substance Use and Overdose Prevention 

Day V – The CES, Housing & Retention, Benefits, and Thriving in the Community

Course Measurement:

This course is measured through a pre and post-test, and participant demonstration of group activities and exercises that reinforce the learning objectives of each module.

Pre-requisites: None


Completion:

Participants will receive an Applied Care Coordination and Systems Navigation Certificate upon completion of the 5-day course. 35-hour CEUs are available for this course from BRN, CAADE, CAMFT, CCAPP.

Who should attend: 

Homeless services staff who have worked at their agency more than 6 months.
