 Making Healthcare Better & Reducing Readmission Rates Through Hospitalists, Technology and Communication

Florida ranks as one of the highest states in readmission rates in the nation. In this presentation we will gain better understanding of how we can rectify this problem and make healthcare better for all by using hospitalists, technology and better communication. Dr Krishan Nagda and his colleagues will show how proper implementation of these processes is reducing the readmission rates by optimizing transitional care between hospitals, skilled nursing facilities and primary care physicians.

Presentation Speakers:

Dr. Krishan Nagda graduated from the University of Louisville in 1996.  He completed his residency at the University of Kentucky in 1999.  In 2001 Dr. Nagda founded Central Florida Inpatient Medicine, which is currently one of the largest independent Hospitalist groups in the state of Florida.  The practice has grown from two providers to over 75+ providers in an integrated health delivery network that has over 300,000 patient encounters in 2014.

Dr. Nagda’s engineering background has contributed greatly to the technological advances that CFIM has been able to make over the past several years in regards to connecting all the different entities involved in a patient’s hospital stay.  CFIM’s comprehensive communication tool was founded on his basic principal that communication is key to efficient and high quality patient care.
 
Dr. Nagda’s focus for the past few years has been building strong relationships with ACOs, Medicare Advantage Plans and their associated IPA/MSO’s that are in the Central Florida market.  Implementation of thorough communication processes allowing for optimal transition of patient care between the hospital, skilled nursing and primary care offices has reduced re-admissions to less than 10%.
 
Dr. Nagda has also served as the Chief Medical Officer for Medicare Advantage Health Plan. Currently he also is the founder & CEO of Advanced Inpatient Solutions a leading provider of work flow inpatient and post-acute care providers.

Patrick Chunn, CEO/CFO – Patrick has over 30 years of healthcare experience as an owner, operator and C-level executive of several successful healthcare organizations. As the CEO/CFO of Golden Health, he provides strategic leadership with several corporate ancillary provider and vendor relationships, our Board of Directors and various sub-committees, and capital partners.  He is also responsible for finance and accounting, financial planning and analysis, and corporate reporting.  He has held C-level positions at several successful middle market healthcare services organizations ranging in size from start-ups to over $1 billion in annual revenue including PatientMatters, Hospital Physician Partners, Independent Living Systems, MHM Services and Vista Health Plans.  Mr. Chunn is a CPA, MBA and CMA.
