Join us at the Healthy Transitions Colorado Kickoff  event to hear from key leaders across the state including keynote speakers US Senator Michael Bennet, Eric Coleman, MD, MPH, and Jane Brock, MD, MSPH, and learn how you or your organization can pledge to participate in this important statewide initiative.

Healthy Transitions Colorado is a collaborative initiative focused on aligning and accelerating existing care transitions efforts, advancing shared goals, and promoting best practices across the state. HTC helps break down the silos of individual facilities and programs to foster true community care coordination across facilities, specialties, and practices with a focus on improving the health and lives of Coloradans.

By July 2015, Healthy Transitions Colorado aims to:

1. Prevent 8700 all-cause hospital readmissions within 30 days of discharge (Better Care)
2. Increase patient days outside of the hospital to 34,800 (Better Health)
3. Save over $80 million in health care dollars (Lower Costs)

Map and Parking Instructions:

Campus/Parking Map. Parking is $1/hr. Cards are accepted, but cash is preferred. 

Please parking in the Kiowa lot, or the Julesberg lot or the East parking lot (directly east across the street from the Kiowa lot – unmarked area on map) if Kiowa is full. Parking attendants will be on hand to expedite parking, and signs will be available from Colfax directing people to the parking lots. From the lots, there will be Healthy Transitions Colorado signs leading to the building.
