Essential Elements of Home-Based Primary Care

HCCI is at the forefront of training professionals in home-based primary care, developing the first-ever education curriculum that explores the full spectrum of home-based primary care: medical, social and economic.  This program is designed for both clinicians and practice managers, consists of classroom training, mentorship, mini-fellowships and online learning opportunities.

Learning Experience 1: Workshop (Classroom): participants receive 12 hours of classroom education at the Center of Excellence (COE) facilitated by medical school faculty members.  Focuses on clinical and economic factors unique to home-based primary care.

Learning Experience 2: Mentorship: Following classroom training, participants move into an ongoing mentorship for one year, including webinars and online discussions/Q&A facilitated by expert faculty.

Learning Experience 3: Mini-Fellowship: Upon completion of the mentorship hours, HCCI will connect participants with a home-based primary care liason to schedule a 2-day "Mini-Fellowship" to accompany a highly skilled home-based primary care provider.
