Please join us for coffee & conversation!  Come enjoy a cup of coffee and meet Shannon Choate, LCSW - Clinical Director & Faith Hooks, Community Resource Specialist of Choices Coordinated Care Solutions.  

Choices is the State Medicaid appointed Wraparound Agency selected to manage theCoordinated System of Care (CSoC) program in SWLA. Choices provides a care coordinator for each child and family they serve. The care coordinator helps build a custom plan for each youth and family.
