2013-2014 Block Membership Purchase Form 

NBS membership can be purchased individually or as part of a chapter block purchase.  All applicants must fill out the individual membership registration form.  The chapter advisor or president must also fill out this Chapter Block Membership Purchase form.

Chapters may purchase a block of 20, one-year, memberships for $500.  Payment will be made with one check or credit card charge.  The names of members covered in the purchase will be listed below.  In addition, each member must submit an individual membership registration form.  The memberships will be valid from the time of submission until November 1, 2014.  (If there are less than 20 members at the time this form and payment are submitted, the chapter president or advisor may authorize the addition of members as the one year period progresses.  However, the memberships of all included in this purchase will expire on November 1, 2014.)  This purchase includes both student and professional members who are affiliated with a chapter.

Former members whose membership has been expired more than 15 days must also pay a $25 membership reactivation fee.

Former members and renewing members should include their membership number in the application process.  The membership number can be found on your membership card and certificate or on the website at www.nbs-aerho.org.  (Look under the Join/Renew button.)

New and reactivating chapers may purchase a block of 10 memberships for $300 in their first year of operation.  To do so, you will have to enter the code provided by the national office in the registration form.

Students and professionals who are not affiliated with a college that has an NBS chapter, may join the National Chapter on an indivual basis.
