Nelson Johnson Memorial Golf Invitational 2012

Come on out for a fabulous day of golf at Whitevale, a private course in the town of Whitevale near Pickering and raise money for Broad Reach. This will be our 8th year for the event, which is held in honor of Nelson Johnson, an avid golfer before he passed away far too prematurely.

$175 per Golfer; $150 Early Bird before July 13, 2012.

Registration fee includes the round of golf, a power cart and a steak dinner. For those who do not want a steak dinner, please choose the vegetarian option on the registration form.

Registration from 11:00 a.m. Shotgun start at 1:00 p.m.

A BBQ lunch will be available between 11:00 a.m. and 1:00 p.m. at a very reasonable cost.

• Register as an individual or as a team
• Tournament limited to the first 120 Golfers
• Please include on the registration form all guests for whom you are paying
• Anyone making ANY payment—golf fees and/or donation—must complete a registration form for your tax receipt
• Please add names in the space provided of playing partners for whom you are NOT paying
• Registrations will only be confirmed after payment is received
• A tax receipt of $50 (plus any donation amount) will be issued to the Primary Name for each registrant

Prize donations are welcome to expand our prize table for golf prizes, draws and raffles. Tax receipt will be issued for all sponsorship opportunities.
