Shoe Distribution will take place

Please Read the information below carefully before registering.

*Please fill out the registration form in its completely. If any requested information is missing it may delay or disqualify your registration.

*Be sure to list your child's name, sex and age.

*Please register each child only once.

*Double check to make sure you have selected the right shoe size.

*If your child's shoe size is not listed, we are out of that size.

*Only 4 children per household.

*When you come to pick up the shoes, children must be present.
