Four years ago, we brought the community together to address the challenges that are faced as professionals and families around advance care planning. We determined we needed to work together and look at a new way to engage individuals and families. Since then, we have developed instrumental local, state and national partnerships and implemented valuable education across our community.
The goal of the Quality of Life Care Planning Initiative on Cape Cod is to assist health care providers, professionals, community organizations, and individuals to improve how we share goals, values and preferences across care settings. Join us for a discussion around how we can work together to make a difference before the time of a health crisis and plan for quality of life at all stages of life.
