Quality of Life Summit 2019

"A good day starts with a good talk."

 

Four years ago, we brought the community together to address the challenges that are faced as professionals and families around advance care planning. We determined we needed to work together and look at a new way to engage individuals and families.  Since then, we have developed instrumental local, state and national partnerships and implemented valuable education across our community. 

 

The goal of the Quality of Life Care Planning Initiative on Cape Cod is to assist health care providers, professionals, community organizations, and individuals to improve how we share goals, values and preferences across care settings.  Join us for a discussion around how we can work together to make a difference before the time of a health crisis and plan for quality of life at all stages of life.

 

Why You should attend -  Learn the value of your role by engaging individuals, caregivers and families in conversation about "what matters most"  and being better prepared before time of crisis.  Also, network with other professionals who are part of these conversation across the community

• Speakers will present  their research results  and how the Massachusetts Coalition for Serious Illness Care (MCSIC) and their partner organizations like Cape Cod Healthcare who are leading the way to develop a new model in how engage in conversation and planning

• A panel discussion with leading local experts will address serious illness care, dementia, financial and legal planning.
