Transportation Services

Located in the heart of Downtown Grand Rapids at the crossroads of both US-131 and I-96, the hotel is approximately a 20-minute drive from the airport, Gerald R. Ford International and easily accessible.  This will require the use of a shuttle, taxi, or rental car to travel to the hotel.

The airport is served by five passenger airlines with nearly 120 nonstop flights to and from 24 major market destinations.  Daily flights from American Airlines, Delta, Southwest, United, and Allegiant. The National Center for Farmworker Health, Inc. cordially invites all professionals throughout the field of agricultural worker health to our annual conference in vibrant Grand Rapids, MI.

The 27th Annual Midwest Stream Forum for Agricultural Worker Health, produced by the National Center for Farmworker Health, Inc., is an annual conference that provides quality, agricultural worker specific, professional development experiences for individuals in all disciplines within agricultural-worker health. The Forum offers a diverse program featuring sessions on clinical topics, lay health outreach, research, and leadership development. Participants will have the opportunity to obtain continuing education credits. Join your colleagues in Grand Rapids, MI for information-packed sessions.

Who Should Attend…

Health care providers, administrators, board members, health educators, lay health professionals including Community Health Workers and Promotores(as), students, researchers, agricultural workers and co-sponsors are invited to assemble and share new ideas, learn innovative strategies, and enhance their skills in health care delivery for the migrant and seasonal agricultural worker population. Individuals and organizations working with the Agricultural Worker community, as well as rural populations and/or other vulnerable populations, will find benefit in the educational content offered. With the purpose of supporting champions in increasing quality and access to care for Agricultural Workers, the Forum emphasizes the exchange of information on model programs, new resources and tools, and the latest research and trends in agricultural work.

Continuing Education Credits (CEUs) will be available through recognized accrediting bodies in Nursing (CNE), Medical (CME), Social Work (CEU), and Texas Community Health Workers (non-certified DSHS CEUs). CEUs are currently being requested from the appropriate specialty organizations. Applying for multiple disciplines is accepted and applications are free.

Each paid registrant will be provided with a conference program and bag, admittance into all workshops, discussion groups, a Welcome Reception, a Lunch Keynote presentation on Wednesday, a daily breakfast, snacks, and beverages.

We are excited to be offering group discounts this year! 3 registrants or more from the same organization qualify for a discounted registration fee of Early Bird $250 and Standard Fee (after September 1st) of $300. Please note: presenters, co-presenters receiving a discount, and individuals receiving Training Stipends are not eligible to be counted toward the discounted registration.
