This webinar will enhance your understanding of error and provide an introduction to Human Factors Science. Proactive system safety assessment will be discussed. 

Objectives:

1. Participants will understand the difference between active and latent failures and how they contribute to threats to patient safety.

2. Participants will understand the evolution in thinking about safety events in the aviation industry and how that evolution can benefit safety in health care. 

3. Participants will be able to identify common human factors vulnerabilities in care processes.

4. Participants will be able to explain how Dana-Farber Cancer Institute developed a comprehensive methodology to conduct system safety assessments and applied it to reduce risks to patient safety.
