All returning Discipleship Ministry Leaders (who are 18 and older) need to fill out this Release Form in the years you are not required to have a background check. If you are a veteran leader and your last background check was two years ago, you need to complete the "Leader Release Form With Background Check” found at www.familyfaithlife.org/leader
