Objectives: 
1. Appreciate the benefit of end of life discussions for patients, families, providers, and systems.
2. Apply a structured approach to goals of care conversations
3. Recognize the role of hospice in improving quality care and decreasing health care cost
4. Identify circumstances when it is necessary to call the medical examiner 
5. Identify circumstances when the physician should sign the death certificate
