OCIM/SPARC Exhibitor and Sponsor Application 

Thank you for supporting the OCIM/SPARC Conference 2015!

Approximately 150 (and growing!) health providers and practitioners, researchers and students representing a wide variety of health disciplines attend the OCIM/SPARC Conference. By becoming an OCIM/SPARC Conference 2015 sponsor, you can connect directly with local health care providers and researchers to promote your products or services.

Sponsorship at OCIM/SPARC Conference 2015 enables us to put on a high-quality scientific conference, it provides dedicated training for students and new investigators and scholarships for those who find it cost prohibitive to attend. It provides opportunities for our attendees to learn about integrative health services and products.

TYPES OF SPONSORSHIP

Educational Support Grant
$350 - Acknowledgement of support in the conference brochure and program

Exhibitor
$500 - Exhibitor space and acknowledgement in the conference brochure and program

Wine and Cheese Reception Sponsor
$1,000 - $5,000 - Exhibitor space, acknowledgement of support in the conference website, brochure and program, acknowledgement at reception, promotional flyer included in attendee packet
Other Donation

ADDITIONAL INFORMATION FOR EXHIBITORS:
Each Exhibit table Includes:

• One 6’ table with two side chairs
• Two Exhibitor name badges*
• Listing in all exhibit acknowledgment areas of website and onsite materials
*Exhibitor names badges allow access to the exhibit floor space, but does not include conference meals or access to sessions. Full conference registration can be purchased separately unless the Wine and Cheese Reception Sponsor level is chosen.

SELECTION OF EXHIBITORS:
Only firms and organizations whose services and products are appropriately related to the purpose of the OCIM/SPARC Conference shall be permitted to exhibit. OCIM reserves the right to decline or prohibit any exhibit which in its sole judgment is inappropriate, this reservation being all inclusive as to person, things, printed matter, products and conduct.

CANCELLATION POLICY
No cancellation by an Exhibitor is effective unless given in writing by mail, fax or e-mail to the address listed above or info@o-cim.org. Exhibitors canceling on or before April 15, 2015 will be subject to a cancellation fee equal to 50% of the total cost of cancelled space. Cancellations occurring after April 15, 2015 are subject to a cancellation fee equal to 100% of the total cost of cancelled space. All outstanding cancellation fees are payable immediately upon cancellation.
