                                                                     DESCRIPTION
Do you wonder what forms of assistance are available to seniors in your area?  Would you like to learn more about programs administered through your local Area Agency on Aging?  Don’t miss this short and informative session.  Local Area Agencies are your one-stop for information, assistance, and referrals for seniors! Our planning and service area includes:  Ashland, Crawford, Huron, Knox, Marion, Morrow, Richland, Seneca, and Wyandot counties.  You will about funding sources, how they are disbursed, and about programs associated with the Older Americans Act, administered through the Administration on Aging via the Ohio Department of Aging and ultimately the Area Agencies on Aging.

                                                                    OBJECTIVES
Upon completion of this training, participants will be more familiar with the following programs and services administered by the Area Agency on Aging:

  • The Older Americans Act
  • The Ohio Department of Aging
  • Federal, State & Local resources
  • Programs/services administered through our Area Agency on Aging:

PASSPORT - RSS - LTC Ombudsman - Caregiver Assistance - Housing - County Focal Points

HEAP - Community Care Coordination - HEMEN - Training

                  PRESENTER:  Teresa Cook, Chief of Marketing and Development
Teresa Cook is the Vice President of Marketing and Development at the Ohio District 5 Area Agency.  She has worked in social services for over 21 years and has spent 18 years with the Area Agency on Aging working to provide resources and quality care to individuals over age 60 and those with disabilities.  She has a comprehensive background in aging services and supports including marketing, development, service coordination, public speaking, and project management.  She has written and administrated federal and state grants for senior hunger and senior home repair and minor home modification.  Her research, programming, and innovative outreach strategies for rural aging population has been recognized at national conferences on food insecurity, SNAP, and senior hunger.  Teresa is a graduate of the Disney Institute of Business Excellence.  She is a Certified Information and Referral Specialist for Aging/Disability and is a Master Trainer for Stanford University's Chronic and Diabetic Self-Management Programs and for Matter of Balance falls prevention curriculum.

                                                           CONTINUING EDUCATION
LSW and LISW:  Approved by the CSWMFTB (MSX091704) for social work continuing education for 2.25 CEUs.  Nurses may use continuing education credits approved by the CSWMFTB for social workers to meet their CNE requirement.  All others will receive a certificate of attendance for 2.25 clock hours.

                                                       REGISTRATION AND REFUNDS
The deadline for registration is December 7, 2017.  Registration is not complete without payment or invoice information.  Late registrations are accepted if space is available and will include a $10 late fee A refund will be made ONLY for cancellations received five work days prior to the training.  If received less than five days prior to the training, a credit can be issued for use at another training within the next year.  If notice of cancellation is not received before the date of the training, no refund will be issued.
