FORE THE KIDS Golf Tournament

IMPORTANT EVENT INFORMATION:

The 6th Annual FORE THE KIDS Golf Tournament provides entertainment for golfers of all abilities who come together to support the Rochester Area Family YMCA.  

All proceeds from this event benefit the Y's Open Doors Scholarship program.  The Y believes that all kids should have open access to Y programs and activities regardless of their ability to pay.  In 2016 alone, the Y provided 2,704 people with financial assistance for Y programs to build healthy spirits, minds and bodies for all.  

Please join us on September 13th at Eastwood Golf Course and help us make more possible for children in our community.

REGISTRATION FEES: includes 18 holes of golf, cart, lunch and dinner.  Golfers will be grouped in teams of four.  Please indicate on registration the individuals you would like to team with.  Each golfer is required to register sparately.  

• EARLY BIRD SPECIAL (through August 31):  $100.00 per golfer 
• AFTER AUGUST 31:  $125.00 per golfer

• Eastwood Golf Course, 3505 Eastwood Rd SE, Rochester, MN  55904
• Noon - Lunch.  Menu is Pulled Pork, Beans, Potato Salad and Chips.
• 1pm - Shotgun Start: Find your team's carts and head out to your assigned hole!
• 5:30pm - Dinner & Presentation.  Menu includes taco bar, dessert and cash bar.  Presentation will include a mission moment from the Y, thanks to sponsors and prizes!

YMCA FORE THE KIDS Golf Tournament 2017 Waiver and Release

In consideration of being permitted to utilize the facilities, services and programs of the Rochester Area Family YMCA (“YMCA”) for any purpose including, but not limited to, observation or use of facilities and equipment and participation in any program or event affiliated with the YMCA without respect to location, I, on behalf of myself and any children, dependents or personal representatives, hereby: 1. Acknowledge that I have (a) read this release and waiver of liability; and (b) voluntarily sign this release and waiver of liability. 2. Release the YMCA, its directors, officers, employees, agents and volunteers (collectively “YMCA Releasees”) and the City of Rochester, its directors, officers, employees, agents and volunteers (collectively “City of Rochester Releasees”) from all liability to me for any loss or damage to property or injury or death to person, whether caused by the ordinary negligence of the YMCA Releasees and City of Rochester Releasees or any other person, and while I am in, upon or about any YMCA branch or any facilities or equipment therein or participating in any program or event affiliated with the YMCA. 3. Agree not to sue the YMCA Releasees or City of Rochester Releasees for any loss, liability, damage, injury or death described above and I agree to indemnify and hold harmless the YMCA Releasees and City of Rochester Releasees and each of them from any loss, damage or cost they may incur due to my presence in, upon or about any YMCA branch or any facilities or equipment therein or my participation in any program or event affiliated with the YMCA whether caused by the ordinary negligence of the YMCA Releasees, City of Rochester Releasees or by any other person. I assume full responsibility for the risk of such loss, liability, damage, injury or death. I intend for this release and waiver of liability to be as broad and inclusive as is permitted by the laws of the State of Minnesota. If any portion hereof is held invalid, I agree that the balance shall continue in full force and effect.

I grant permission to the Rochester Area Family YMCA to use my photographs, motion pictures, recordings or any other record of this event for any legitimate purpose.
