Photo ID is required to enter the Federal building and allocate sufficient time to go through the security check-point when you enter the Federal Building.

Take the elevators to the 6th Floor Conference Center.


 

Travel Directions

By Subway

  • 4, 5, or 6 trains to the Brooklyn Bridge/City Hall Station. Exit the station and walk ½ block north on Lafayette to Duane Street. Left on Duane Street to Broadway. Enter at the  Visitor’s Entrance
  • A or C trains to Chambers Street. Exit the station and walk north on Church Street 2 blocks to Duane Street. Make a right on Duane Street to Broadway. Enter at the Visitor’s Entrance
  • N or R train to the City Hall Station. Exit the station and walk north on Broadway 3 blocks to Duane Street. Enter at the Visitor’s Entrance
  • 1,2,3, or 9 train to Chambers Street. Exit the station and walk north on West Broadway 2 blocks to Duane Street. Make a right on Duane Street to Broadway. Enter at the Visitor’s Entrance

 From Penn Station:

  • A, C, 1,2,3, or 9 train to Chambers Street and continue as above

Parking: Available at local parking lots on  Duane and Worth Streets at approximately  $35-$45 per day

 

 

 On behalf of the Region II U.S. Department of Health and Human Services' Centers for Medicare and Medicaid Services (CMS) and the Office of the Assistant Secretary for Health (OASH) we cordially invite you to join us for a free one day meeting focused on prevention and chronic disease management.

 This one day meeting, Bringing the Affordable Care Act Home: Linking Communities with Prevention and Chronic Disease Management will inform and engage stakeholders on the impact and improvements made possible by the Affordable Care Act for prevention, treatment and care services in New York City.

Please join us on Thursday, January 24, 2013 from 8:00 AM to 5:00 PM, at the 6th Floor Conference Center in the Jacob Javits Federal Building located at 26 Federal Plaza, New York, N.Y. (downtown Manhattan). 
 

OBJECTIVES

  • To bring together various groups impacted by chronic conditions from across the five boroughs and develop a network for improving prevention, treatment, and care services in New York City.
  • To share information and resources, highlight changes in the health care system that will benefit people living with chronic conditions, including new models of care set in place by the Affordable Care Act.
