CHUSY Region TO 5k and 3 on 3 Basketball Tournament 

The TO 5k and 3 on 3 Basketball tournament costs $36 which is payable online with credit card. If you want to pay by check, please fill out the application online and print it out and send it in with your check.  We will have no other record of your registration without the print out.
