Please complete the following registration to request to be a speaker at the CDAHCC Expo on October 31.

Applicant Guidelines:

• You must have a booth at the fair to be a speaker.
• The application to be a speaker does not mean automatic acceptance.
• Applications will be selected according to group interest, educational opportunity and variety of speakers.
• Please submit this form along with your booth application no later than September 15, 2015
• You will receive notification of your application status no later than October 15th, 2015
• Speaker time is 25 minutes.
