POLST Training for Providers in Hawai‘i

Kokua Mau, Hawaii Hospice and Palliative Care Organization invites you to a 4 hour training for providers who are completing the POLST.

Who should attend: All Professionals especially providers who can complete and sign the form - Advance Practice Registered Nurses (APRNs) and Physicians. 

The workshop includes reviewing the POLST orders, initiating POLST and Advance Care Planning (ACP) conversations including ACP decision video aides tools and role-playing. The faculty are experts from our Hawaii community, all with direct experience of discussing and completing POLST. What is POLST?
In 2014 POLST (Provider Orders for Life-Sustaining Treatment) legislation was passed giving signing privileges to APRNs and Physicians for a POLST. The Kokua Mau website is the central source for POLST information for Hawai‘i.

POLST is a portable medical order that gives patients more control over their end-of-life care.  It specifies the types of treatments that a patient wishes to receive towards the end of life. Completing a POLST form encourages communication between healthcare providers and patients, enabling patients to make more informed decisions. POLST form documents those decisions in a clear manner and can be quickly understood by all providers, including first responders and emergency medical services (EMS) personnel. As a result, the patient’s wishes can be honored across all settings of care.
