SESSION II: COMMUTER OPTION
In addition to registering for the JOHN THOMPSON III BASKETBALL SCHOOL, each participant must submit completed versions of the Assumption of Risk/Waiver of Liability/Parental Permission Form and the accompanying Health Form linked below. Participants who have not completed both forms will not be permitted to participate in camp activities until they are received.
